Human Error

Human error refers to the failure we all encounter from time to time to attain our goals. These can be failures of planning or knowledge ("mistakes"), failures to execute explicitly or implicitly intended actions ("lapses"), or faulty motor outputs ("slips", Reason, 1990). Often these errors have complex, compounded causality intimately tied to the human operator's task environment, including their procedures and the tools they have to perform those procedures.
